Insurance

I am an out-of-network provider and can provide you with documentation should you wish to submit it to your insurance for reimbursement.  I also accept HSA/FSA cards. Forms

I use a secure HIPAA compliant portal for my forms and scheduling. After a brief phone consultation, you will be provided with information to log into the portal to complete your forms online.
